If you're still in the process of selecting a clinic that best fits your needs, please look below for answers to questions to facilitate that decision. You can also give us a call and we would be happy to specifically answer questions about your insurance coverage and physical therapy benefits. We are currently in the process of becoming an in-network provider for most major insurers. As that process evolves we are currently able to see patients under Medicare and a direct fee for service basis. As a new clinic still contracting with insurers, we don't want finances to prevent patients from receiving excellent care. We are offering 40% off our self-pay rate through the month of August. Preparing for your first visit
As we continue working on becoming an in-network provider for more insurances, please give us a call prior to your first appointment to check your physical therapy eligibility and benefits. To speed up the check-in process, you can download and complete the forms below prior to your first visit and bring them with you.
If you are preparing for an upcoming surgery, we strongly recommend beginning the above mentioned process as soon as you have a surgical date so that we can set up a schedule that works best for you.

Can I insert a video in my FAQ?Yes! Do you accept insurance?To add a new question go to app settings and press "Manage Questions" button.

How do I edit or remove the "FAQ title"?"The FAQ title can be adjusted in the settings tab of the App Settings. You can also remove the title by unchecking its checkbox in the settings tab.
